The Location of Intraocular Lenses in an In-vitro Capsular Bag Model - an Investigation using 3D Ultrasound Bio-microscopy

Purpose: The exact location of intraocular lenses in the capsular bag is one of the most important details corresponding prophylaxis of posterior capsule opacification. The aim of this investigation was to obtain information about the exact positioning of different newer IOL designs and materials (acrylate, silicone) in a capsular bag model.
Method: After implantation of different IOL's (Alcon, Bausch&Lomb, Allergan) in a in a artificial capsular bag model (Nusil, Silicone Technology, Franc), this model system was scanned using 3D ultrasound biomicroscopy. The location of the IOL including haptic in the capsular bag as well as the gap between IOL and posterior capsular bag surface was analysed.
Results: The placement of 8 intraocular lenses with different designs in the capsular bag model was estimated in three dimensions. The central part of all investigated IOL optics is well adjusted to the posterior capsular back surface. The equatorial zone shows different gap sizes (<0.3mm) depending from the IOL design. The alignement of the haptic and the impact to the gap size can be analysed.
Conclusions: The used capsular bag model in combination with the 3D ultrasound biomicroscopy provide information about the skill of different IOL designs. Conclusions corresponding a gap free zone between optic / posterior capsular bag surface and there regarding the posterior capsule opacification can be drawn.
