Influence of Cut-off Filters on Reading-behavior in Age-related Macular Degeneration

Background: Cut-off filters may result in improved contrast sensitivity in different retinal dystrophies and therefore lead to improved visual function. Recently, it has been postulated that a cut-off filter at 540 nm might improve vision in age-related macular degeneration (AMD). We investigated whether cut-off filters result in improved reading ability.
Methods: Reading performance was examined in 22 patients with AMD aged 63 to 91 years (Visual Acuity 0.125-0.4) using the Radner reading charts. These charts were presented in random order either with or without cut-off filters Zeiss CF 540 and CF 580 monocularly to the better eye using a defined illumination of 105 cd/m2.
Results: Using the CF 540 cut-off filter 11 patients improved for 1 line while 11 decreased compared to reading without filters. Calculating the LogRAD-Score, which takes reading errors into account, 3 patients improved who had less reading errors. Seven patients subjectively preferred reading with CF 540 while 1 was really upset. Reading speed improved in 9 patients while it decreased in 13, however all changes were minimal. There was no improvement using CF 580.
Conclusions: Overall, we fvound no measureable improvement using the CF 540 cut-off filter. Even the subjective impression was positive only in few patients. Cut-off filter 580, which was tested for comparison does not show any advantage. In summarization cut-off filters do not improve reading behaviour in age-related macular degeneration.
